Opening a Health Savings Account ("HSA") with First Horizon is quick and easy. You can complete the process online in just a few minutes. Please provide the information on the following screens to open your HSA with First Horizon Bank, a division of First Tennessee Bank National Association, the custodian of your HSA. First Horizon Bank will share this information with First Horizon Msaver, Inc., along with other information as indicated in the Application, to assist First Horizon Msaver in providing HSA administrative services to you.

USA PATRIOT ACT INFORMATION

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify and record information that identifies each person who opens an account. What this means to you: When you open an account we will ask for your name, street address, date of birth and other information that will allow us to identify you.

Items you will need to Enroll

Before you begin the enrollment process, you may need the following:

  • 1. A valid email address to which you have access
  • 2. One form of Identification such as a Drivers License, State I.D., or Passport
  • 3. Your Social Security Number, Date of Birth, and Residential Street Address
  • 4. Optional: If you want to designate a secondary signer such as your spouse, you will need to know that person's Social Security Number, Date of Birth, and Residential Street Address.

This online enrollment form is not for individuals enrolling as members of a group. If you are a member of a group and are trying to enroll, use the Online Enrollment link provided by your group.
